Abigail Hoffman, FNP

 Abigail Hoffman, FNP-C, is a Certified Family Nurse Practitioner, specializing in Family Medicine. She is a graduate of Chamberlain University where she earned a Master of Science - Family Nurse Practitioner. Abigail completed her Bachelor of at Lourdes University and went on to work in a hospital setting in various specialty areas including critical care, OB, and the cardiac catheterization lab. Abigail also served as a clinical instructor at Terra State College. 

Abigail is accepting new patients at the CHS Stony Ridge office in Stony Ridge, Ohio (just  of east of Perrysburg), and the CHS East office in Fremont, Ohio. She sees patients of all ages, both children and adults.

Abigail is community minded and served as a volunteer firefighter while completing her education.  She is also a mom and loves to spend as much time as possible with her family-- especially camping or at the lake.
