Laboratory services are available onsite at the Fremont Main Campus and Stony Ridge Office These services are provided by PathLabs for the convenience of our CHS patient
For CHS self-pay patients, Path Labs honors CHS' sliding fee scale for all lab testing. All laboratory testing must be ordered by your provider.
Prescription Assistance
If you need help paying for your medications, please contact our CHS Patient Care Coordinator at (419) 334-3689. The Patient Care Coordinator will work with you to find assistance based on your insurance, income, and community resources.
