CHS Awarded Four National 2025 Quality Awards!

Community Health Quality Recognition badges are awarded by Health Resources and Services Administration (HRSA) annually to Health Centers across the country, recognizing those organizations that have achieved or exceeded quality benchmarks in:

• Access
• Clinical Quality
• Health Outcomes
• Health Information Technology (HIT)

The badges are awarded based on data used to collect information about health centers' performance and operations.  It includes services provided, clinical processes, health outcomes, patient characteristics, and other factors. This data helps assess the impact and performance of the Community Health Centers, identify trends, and improve the quality of care. 

The four Community Health Quality Recognition Badges awarded to Community Health Services (CHS) this year are a testament to the outstanding care CHS provides, the organization's commitment to continuous improvement, and leadership in leveraging technology to enhance patient outcomes.

CHS was recognized in the following four categories:

National Quality Leader in Heart Health Badge recognizes CHS as a top-performing health center in the nation in cardiovascular care. 

The High-Value Care Badge recognizes CHS for meeting or exceeding the Quality score. The Quality Score is based on health centers’ Clinical Quality Measures (CQM) performance.          

The Bronze Badge for Health Center Quality Leadership recognizes the organization's performance across key clinical quality measures. CHS was ranked in the top 21-30% nationally. 

Advancing Health Information Technology (HIT) Badge recognizes innovation in using data and digital tools. This includes offering telehealth services, exchange in clinical information online with key providers’ health care settings and engaging patients through Health Information Technology.
