How to become a new Dental patient:

If you're a new patient, dental services are available on a first-come, first-served basis at the beginning of each month.
To schedule an appointment, please call (419) 334-8855.

How to become a new Medical patient:

Please click on the link below to complete and submit the secure online Patient Registration form
Please allow at least 15 minutes to complete the registration
Once we have received your completed registration form, we will send you a text with your appointment date and time
