Does Community Health Services (CHS) accept Medicaid and Medicare?

Yes. CHS accepts all kinds of payments, including Medicaid, Medicare, commercial insurance plans, and self-pay patients.  For those who qualify, based on family size and income, CHS also offers a sliding fee schedule or discounted rate.

Does CHS take all other insurance?

We accept most insurance plans.  However, it’s a good idea to check with your insurance company to confirm CHS is included in your network.

Where is CHS located?

CHS has offices throughout Northwest Ohio including; Fremont, Fostoria, Stony Ridge, Lima, Bellevue and Napoleon. We also have a seasonal office located in Willard. Please click on locations on this website to see a map with locations and more information.

How do I become a CHS patient?

The first step is to register as a new patient by filling out the new patient packet that can be found on this website. You can submit it online, or print, and drop it off at the CHS office near you. Once you’re registered, a member of our staff will call you to schedule your appointment, you can also call (419) 334-3869 to schedule an appointment with one of our medical providers, or (419) 334-8855 for a dental appointment.

What is a Provider?

Provider is a term used for all professionals that provide your medical, dental, or behavioral health care. This includes Doctors, Dentists, Nurse Practitioners (NP), Physician Assistants (PA)

Do you take walk-in appointments?

No, we require appointments. This is to ensure that our patients are seen promptly, and have the appropriate time they need with their provider.
