In the late 1960s, Community Health Services (CHS) started as a small, seasonal clinic to provide health care services to migrant workers. It was staffed by volunteers and located in the back of a local pharmacy. We've grown a lot over the years, and we’re proud to continue to provide quality health care to generations of families in the communities we serve.
For 50 years, Community Health Services has been recognized as one of the most respected health systems in Northwest Ohio. You can always expect to receive excellent care and customer service at all of our offices because at CHS you’re not just a patient, you’re our priority.
CHS is accredited by the Joint Commission and is certified as a "Primary Care Medical Home" or PCMH. This special accreditation means that CHS is recognized as a model for primary care excellence.
We have nine offices throughout Northwest Ohio. All of our CHS locations are handicap accessible and interpreter services are available for patients who speak a language other than English, and those who are hearing impaired.
1972 - CHS becomes a regional migrant health care delivery system serving four counties
1978 - CHS opens Fremont Office located at 410 Birchard Avenue, to provide health care services for the entire community
1981 - CHS expands to offer dental services
1999 - CHS is accredited by the Joint Commission demonstrating an ongoing commitment to the highest quality of care
2005 - CHS opens West Side Pediatrics Office
2008 - CHS opens Napoleon Office
2012 - CHS opens Fostoria Office
2012 - CHS expands to offer Behavioral Health Services
2013 - CHS opens Stony Ridge Office
2016 - CHS opens Lima Office
2017 - CHS opens new main campus at 2221 Hayes Avenue
2018 - CHS expands to offer OB/Gyn to our list of services
2020 - Dental services added to our newly renovated Napoleon Office
2020 - CHS opens Third Avenue Office offering OB/Gyn care
