Insurance and Enrollment

What insurance plans do you accept?

CHS accepts all Medicaid, all Medicaid managed care plans, and Medicare.  We also accept almost all insurance plans.  Please verify with your insurance company that CHS and our providers are in your covered network.  It is the patient’s responsibility to verify with their insurance company that CHS and our providers are in their covered network.  Patients are responsible for all out-of-network and non-covered fees.

What if I don’t have insurance?

CHS offers a sliding fee schedule or discount of fees for qualifying patients based on family size and income.  No patient is turned away from a CHS office based on financial status.  Please contact our offices for more information, or to see if you qualify.

If you’re uninsured and meet certain requirements, you may be eligible for Medicaid.  Our Outreach and Enrollment Specialist can assist you in determining your eligibility and submitting an application for Medicaid coverage.  

Medicaid Patients

As a Federally Qualified Health Center (FQHC), CHS is able to accept any type of Medicaid plan.  

Here are a few important things to keep in mind:

  • The primary care provider (PCP) on your Medicaid card may be automatically assigned
  • This does not mean that you are automatically a patient of the provider shown on your card
  • You are required to complete the New Patient Registration process if you have not been seen at a CHS office within the last three years.
Marketplace Insurance Eligibility

Health plans in the Insurance Marketplace are offered by private insurance companies during a specific open enrollment period.  By filling out a Marketplace application, you can find out if you qualify for a lower cost for your health insurance based on your income.

You can only purchase a health care plan from the Marketplace, or make changes to your current plan during a specified open enrollment period unless you have a qualifying life event.  For more information, or to sign up for coverage through the Marketplace visit HealthCare.gov.
