SCOPE OF CARE
Our providers specialize in Family Medicine, Internal Medicine, Behavioral Health and Dentistry. Our organization also offers pharmacy services, including free delivery throughout the services area. In addition to excellent care, all of our employees at each of our medical and dental offices are committed to bringing you the highest standards of customer service.
GOALS OF THE PRIMARY CARE MEDICAL HOME (PCMH)
- Provide access to high quality preventative healthcare along with chronic disease management.
- Improve the patient’s care experience through 24 hour access to care.
- Involve the patient in the treatment plan.
- We will give equal treatment, care and accommodations to all patients regardless of race, creed, sexual orientation orgender identity.
- Provide care to all patients regardless what their payor source or financial situation.
RIGHTS AS A PATIENT
- As the patient you have the right to select your own Primary Care Provider.
- As part of the Primary Care Medical Home you have the right to obtain care from other clinicians within the primary caremedical home, to seek a second opinion, and to seek specialty care.
PATIENT’S RESPONSIBILITIES
- Bring a list of ALL medications with you to every appointment. This includes prescriptions, over the counter medications, and herbal medication.
- Provide any new changes to your medical history which includes: New medical diagnosis, surgeries, and hospitalizations.
- It is the patient’s responsibility to carry out the recommended treatment plan outlined between you and your primary care provider.
REFERRAL PROCESS
To ensure all patients receive referrals to outside providers for specialty care and for all diagnostic procedures, we will coordinate the following;
- All medical referrals for outside specialty care will be provided to the patient at checkout and sent directly to the appropriate specialist.
- Patients are instructed to contact the specialist if they are not contacted by the by the provider's office in a few days.
- Patient Services Representatives (PSR's) follow-up on the referrals to confirm appointment times and obtain reports.
- It is the responsibility of the patient as part of his or her treatment plan to carry out appointments scheduled with specialist/facility or for diagnostic procedures.
