No-Show Policy 

To ensure that all of our patients receive the best possible care on a timely basis, it’s important that everyone keep their scheduled appointments and arrive on time. To review our policy, which includes recent changes regarding canceling your appointment and missed appointments, please click on the link below. 

No-Show Policy

If you feel you have been cited for arriving late or not keeping an appointment unfairly, there is an appeal process. Please click on the “No-Show Appeal Form” link below. You have 14 days from the date of your missed appointment to appeal. You must complete and submit this form online.

No Show Reinstatement class

• To take the Reinstatement Class, click on the Reinstatement Class sign-up link below. 
• Please make sure all information is correct and current. 
• Once sign-up is complete, you will automatically be enrolled in the next class.
• Instructions for completing the class will be sent on the day of the class, via text to the cellphone number you provided.
• You must pass the quiz (link found in the class slides) with an 80% or better score to be reinstated.
• The next online class will be available Monday, June 23, 2025.  Registration for this class closes Friday, June 20, 2025, at 9am.
