On Friday, December 10, 2021, Community Health Services will hold a COVID-19 vaccine booster clinic at our main office, located at 2221 Hayes Avenue in Fremont, Ohio, from 2pm to 4pm. To be eligible for the booster, participants must:
Have received their first two COVID-19 vaccine doses
Received their second dose on or before June 10, 2021
Be 18 years old
