In January 2022, CHS implemented a new electronic medical record system.  This system provides patients with greater access to their medical information, new options for scheduling and tools to track health information for themselves and their families.

The Healow App puts your health care in your hands

Once you’ve signed up for the patient portal, you can download the Healow App, which is a great way to manage your health care anytime, right from your smartphone.  Features include: 

• Access portal features easily from one convenient app
• Access your family’s health care information right from a single app and login
• Check in for your appointments right from your phone
• Our Health tracker gives you the option to track your activity and exercise, blood pressure, and more

Kiosks add convenience and privacy at check-in

In 2022, CHS installed Kiosks in every CHS office.  This new technology provides patients with easy access to check in quickly and privately.  With Kiosk check in you can:

• Quickly check in for your appointment
• Update your personal information
• Pay you bill
• Patient Services Representatives are available to assist if you have any questions 

Schedule your appointments anytime with Online Scheduling

CHS patients can schedule an appointment anytime, night or day, simply by going to our website and clicking on the orange “Schedule Online” button at the top of the website’s home page.  This new feature eliminates the need to wait until our offices open to request an appointment. 

 EXPANSION AND GROWTH IN 2022

In 2022, CHS added a new building on our main campus and a new location to expand our services in the region.

Keen Building Opens on the CHS Main Campus

On April 27, 2022, CHS held the dedication ceremony for the newest addition to our CHS main campus, the Keen Building.  The Keen Building is located behind the main CHS building on the CHS campus located at 2221 Hayes Avenue. 

The Keen Building houses the organization’s call center, professional offices and an additional conference room which is available to non-profit organizations to use for meeting space at no cost.  Organizations may request to reserve this space by contacting Sandy Magnuson at smagnuson@chsohio.com.

The Keen building was named in honor of Mary Keen, a founder of Community Health Services.  Mary Keen was a leader in the organization and worked tirelessly to ensure that the underserved populations in the region have access to quality health care for generations to come.   

New Bellevue Office Provides Family Medical Care and Behavioral Health Services

In October 2022, Community Health Services opened our tenth office.  Through a partnership with The Bellevue Hospital, CHS has expanded our services in the region with an office located at 1255 W. Main Street, Suite B, in Bellevue, Ohio to meet the need for additional providers in that part of the region.

We recruited two excellent providers for this location-- Lucas Shammo, NP-C, specializing in Family Medicine and Eliverta Azizi, PMHNP-C, specializing in Behavioral Health Medication Management.  Both Lucas and Eliverta (Ellie), are accepting new patients.

Like all CHS, we accept Medicaid, Medicare, most insurance plans and self-pay patients at our Bellevue location.  A sliding fee schedule is available for qualifying patients.
