Pfizer COVID Vaccine Clinics Scheduled for Ages 5-11
Vaccines will be given at CHS Main Campus
What you need to know…
- The vaccine clinic location is Community Health Services Main Campus - 2221 Hayes Avenue – Fremont, Ohio
- There are two clinics scheduled for the first dose, they are:
- Saturday, November 13, 2021 • 9am to 3 pm
- Saturday, November 20, 2021 • 9am to noon
- This is a TWO DOSE vaccine. The second dose will be scheduled for 21 days after the first.
- Only children ages 5 to 11 years old are eligible to receive their vaccine at this clinic. Doses for older children, adults, and boosters will not be available.

If your child is covered under any of the health plans listed above, your health plan is offering a $100 Visa gift card when they get their first dose at one of our two clinics in November!
- To qualify, the child must be accompanied by their parent or legal guardian
- The parent or guardian must provide their child’s Medicaid ID number to receive the gift card
- The gift cards will be distributed to the parent or guardian at the vaccine clinic
